Year End Reminders:

Individual Health Plans for the 2014 – 2015 school year for students who require medication in school have been sent home. If you have a student who requires medication in school and you have not received the Health Plan please contact my office.  Please use the summer months to have the plan & doctor’s orders filled out.  Return the plan, WITH the medication, to school on the first day of school.

Medication without the IHP and signed doctor’s orders cannot be accepted.

If you have medication currently stored in the nurse’s office in Westfield or the main office in Mountainside please pick it up prior to the last day of school.  Medication cannot be stored over the summer.  Any medications left in either office will be discarded on the last day of school.

Immunizations: Holy Trinity Interparochial School follows the minimal immunization requirements set forth by the state of New Jersey.  With the outbreaks of Measles, Mumps and Pertussis (Whooping Cough) currently circulating throughout the state, it is critical that all students be up to date on their immunizations.  Notice of the required immunizations for attendance have been sent to all incoming 6th grade students (Jan. 2014) and all incoming Kindergarten students (May 2014) for the 2014 – 2015 school year.

Documentation of all required immunizations for attendance must be in school by the first day of school in September 2014.  There is no grace period on these immunizations and failure to receive or provide documentation of the required immunizations will result in school exclusion until the immunizations/documentation is received at school.

To see a list of the required immunizations for school attendance please visit the HTIS “Health” webpage on the website.

Annual Sports Physicals (Parts A & B) are required for all students in grades 6, 7 & 8 who wish to participate in school sponsored athletics. The physical exam must take place within 365 days of the first practice session in order to be valid.  The Sports Physical form can be found on the HTIS website under “Sports” or “Health”.

Once the sports physical form is received and reviewed in my office,  you will receive a letter stating that your child has been cleared to participate by their physician.

Only after all completed forms have been reviewed will coaches be notified that the student is able to participate.

Please use the summer months to have these physicals done.  The paperwork turn around in the doctor’s office is often 2 weeks or more which will delay your student’s ability to participate in their chosen sport.

Finally, I wish all of you and your families a happy, healthy and relaxing summer vacation.  See you in September!
